House Call Policy – FauntLeRoy Naturopathic Medicine

Eligibility & Service Area

  • House call visits are available for patients within 15 mile radius (depending on location) of 12032 3rd Ave. NE Seattle WA 98125.

  • Patients must be established with FauntLeRoy Naturopathic Medicine or have completed a new patient intake and had a telemedicine consultation prior to scheduling a house call.

  • House calls are intended for non-emergency medical care. If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room.

 

 

Services Provided

House call visits may include, but are not limited to:

​

  • Comprehensive naturopathic assessments

  • Chronic disease management (e.g., diabetes, cardiovascular health)

  • Lifestyle and nutritional counseling

  • Physical exams and basic diagnostic evaluations

  • Supplement and medication management

  • Lab orders and review of results (blood draws may need to be performed at a lab facility) – PENDING: Blood draws will be implemented at a later date.

 

Fees & Payment

  • A house call fee of $50-150 (depending on distance/time traveled) applies in addition to the consultation fee.

  • House calls may be billed to insurance if applicable, but the house call fee is typically not covered by insurance and must be paid out-of-pocket.

  • Payment is due at the time of service. We accept Regence, Premera.

 

Scheduling & Cancellation Policy

  • House calls are by appointment only and must be scheduled at least 2 days in advance.

  • Same-day requests may be accommodated on a case-by-case basis, with an additional expedited service fee of $50.

  • Cancellations or reschedules must be made at least 24 hours in advance. Late cancellations or no-shows may be subject to a cancellation fee.

 

Safety & Consent

  • House calls must be conducted in a safe and professional environment. If at any point the visit is deemed unsafe, the provider reserves the right to leave.

  • A responsible adult (18+) must be present for any visits involving minors or individuals requiring assistance.

  • Patients must sign a house call consent form before the visit.

 

House Call Fee Structure – 

In addition to the standard consultation fee, a travel fee applies based on distance from traveled from our office (12032 3rd Ave. NE, Seattle, WA)

🚗 Base Fee (0-10 miles) – $50

Covers neighborhoods like Northgate, Lake City, Roosevelt, Ballard, Fremont, Greenwood, Phinney Ridge, Wallingford, and Shorline.

 

🚗 Mid-Range Fee (10-15 miles) – $75

Covers areas like Capitol Hill, West Seattle, Ravenna, Magnolia, Queen Anne, Lake Union, Downtown, and Lynwood, Kenmore, Lake Forest Park.

 

🚗 Extended Range (15+ miles) – $100

Covers Bellevue, Renton, Burien, Redmond, Edmonds, . Availability is limited.

 

🚗 Beyond 20 miles – Case-by-Case

  • Custom pricing is based on distance and estimated travel time.

  • Typically starts at $150+ depending on the location.

 

Additional Fees

  • Same-Day/Urgent Visit Fee – Additional $50 (if availability allows).

  • Evening & Weekend House Calls – Additional $50 surcharge.

  • Missed Appointment/Late Cancellation (less than 24 hours' notice) – $50 cancellation fee.
